Sign up from the form below and our Patient Care Coordinator will call you and answer any questions. We will find out how many guests will be attending your shower to send you gift registry cards. Your guests will call us to purchase the gift certificates. After your shower just call our office and we will get started on getting you that wedding day smile you have dreamed of!!

You can purchase gift certificates in the amounts of $25, $50, $75, $100 or more! We take Mastercard and Visa for payment over the phone or you can stop by our office to pay by cash or check.
